I'm being told that they may suggest that my mother receive 24 hour care upon discharge. I don't know if this is possible. She wants to go home so badly. We have a large family and we all live minutes away from her. The thought of a nursing home makes me ill. Is it unreasonable to think that we can take her home? She will need minimum assistance w/her transfers.

Might be able to set up a schedule that works for all of you if you can have folks (Family or a paid caregiver) around when she needs to transfer if she can be alone the rest of the time. Many home care companies have caregivers you can pay for non skilled needs like transfers. But if she needs 24 hour supervision then need to make sure it is around the clock so she is safe. And the med alert button in case of emergencies.
